放射線治療部

放射線治療部は、新病院移転に伴い2017年3月末に治療を開始しました。

基本方針

1. キャンサー・ボード

乳腺、肺、頭頸部、リンパ腫、泌尿器など疾患領域毎に、外科や内科等を含めた合議により、放射線治療の適応を個別に検討し、協働して診療を行う

2. 品質管理

高精度放射線治療 (強度変調・画像誘導放射線治療、定位放射線治療) の品質管理を放射線治療専門医、医学物理士、放射線治療専任技師らによって綿密に行う

3. 多職種介入による毒性管理

看護師、薬剤師、栄養士、歯科衛生士、臨床心理士、社会福祉士などの多職種コメディカルと治療開始前から連携し、強度の強い頭頸部や肺などの根治的化学放射線療法などにおける栄養・疼痛・感染管理や療養環境の維持・改善を円滑に行う

4. 短期緩和的放射線治療

高精度放射線治療を駆使して、遠隔地からの通院が多い沖縄県の特性に合わせた短期間 (週1～2回、計1～5回) の治療を積極的に行う (多標的同時脳定位照射、脊髄打ち抜き脊椎定位照射など)

当院の治療機器はVarian社製True BeamⓇで、6軸可動寝台 (Perfect PitchⓇ) により治療のセットアップ精度を数ミリ (脳、骨では1ミリ) 以内に再現できるため、従来に比べてさらに高精度に病変に放射線を集中させることができます。

取り扱うおもな疾患と治療方法

乳癌

従来の標準的術後照射に加え、内胸・鎖骨上・腋窩リンパ節を必要に応じてVMAT (volumetric-modulated arc therapy, 強度変調回転放射線治療) を用いて均一に照射し、肺や心臓への影響を軽減しています。

肺癌

早期肺癌の約80％で、定位照射による腫瘍の消失が得られています。またリンパ節転移のある進行非小細胞肺癌では、化学療法と放射線治療の同時併用後に免疫チェックポイント阻害剤を用いた方法 (Pacific Regimen) を積極的に用い、世界標準にほぼ匹敵した生存率が現時点では得られています。これらの治療が可能かどうかは、キャンサー・ボード等で慎重に判断されます。

頭頸部癌 (特に喉頭温存治療)

あらゆるがん治療の中でも毒性の強いもののひとつである頭頸部癌の化学放射線治療では、経験豊富なスタッフの多職種介入により安全な完遂を目指しています。切除不能あるいは喉頭全摘を要する局所進行頭頸部癌のうち、2年以上経過観察した20例での治療完遂率100％、2年生存率85％、2年頭頸部無再発率89％の成績が得られています。一人でも多くの声を失う手術を要する方々の喉頭温存を目指しています。しかし臓器機能やその他基礎疾患の状態によっては、化学放射線治療の対象とならない場合もありますので、ご相談下さい。

脳転移

全脳照射は脳内再発率の低下に寄与しますが、数ヶ月後に認知機能低下が生じ得ます。これは海馬保護によって軽減できることが示されています。定位照射は、画像上見えている転移巣の長期制御がより期待できる方法です。評価可能例での奏効率 (がんの縮小が得られた割合) は約8割です。2～4個の多発転移では、これらに対して10分程度で同時に定位照射が行えます。また県内で初めて導入されたHyperArcⓇによって、同時多標的定位照射がさらに短時間で行えるようになりました。
当科では定位照射±海馬保護全脳照射を脳転移治療の基本方針としていますが、がんや全身の状態等に応じて個別に選択されます。

脊椎転移・骨転移

1～5回 (週1～3回/1～2週) の定位放射線治療によって、これらによる癌性疼痛の症状緩和率は約80％、脊髄麻痺の評価可能例での改善率は約56％です。麻痺出現から遅くとも48時間 (可能なら24時間) 以内に治療を開始することで、麻痺の改善率はさらに高くなるとされています。

その他のがん

過去の疾患部位別患者数、治療件数、緩和治療の内容を表に示します。下部直腸進行癌に対する術前化学放射線療法、前立腺癌に対する放射線治療では全例で、また様々ながんに伴う症状の緩和などにもその8割以上でVMATを用いて、有害事象 (副作用) が少なく、なるべく短期間での治療を行っています。

すべての治療において、事前に外科、内科、放射線治療医と患者･家族の皆様とのあいだで、治療の選択肢や内容について十分な説明と合意 (informed consent, IC) を得ることは非常に重要です。当院ではICを重視しており、また初診時からの多科協同・多職種介入で、個別に最適な療養環境が得られるようにつとめています。
